New Enrollment MHHS Enrollment Process for New Students

FILL OUT "NEW ENROLLMENT FORM"

BE AWARE: THIS IS NOT FOR SMUSD CURRENT STUDENTS. (STUDENTS COMING FROM OUR MIDDLE SCHOOLS AND MHHS STUDENTS PASS THROUGH REGISTRATION not enrollment.)

When you finish with the Enrollment process and approved, a  Counselor will contact you to set the student's schedule.
Details about books and ID pictures, etc. will be explained during your communication with the Counselor.

NEW STUDENT ENROLLMENT

Welcome to SMUSD! All new student enrollments will be processed electronically for the 2020-21 school year. Click here for a detailed overview of our New Student Enrollment Requirements.

1. Complete and return (via email) this fillable PDF enrollment packet:

2. Email the following required documents with your completed enrollment packet:

  • Proof of Age
  • Proof of Immunizations
  • Proof of Physical Health Exam
  • Proof of Dental Health Exam
  • Proof of Residency - Two (2) documents to verify your residency
  • Transcript, withdrawal grades, or most recent report card
  • If applicable, most current IEP or 504 Plan 
  • If applicable, check-out form from previous school

Please note we are unable to process your child's enrollment until ALL required documents are received.
