Mandated Vision and Hearing Screening 

The California Department of Education requires Mandatory Vision and Hearing Screening of all students in Kindergarten, 2nd, 5th, 8th and 10th grades.  Because vision and hearing greatly impacts a student’s ability to learn, the State of California requires these screenings to ensure that every student is able to fully access his/her education.  

The SMUSD District Nurses will be performing Mandated Vision and Hearing Screening at your child’s school next week.  If your child does not pass the vision or hearing screening, you will be notified by letter. If your child passes, you will not be notified.  If you do not want your child to participate in the Mandated Vision and Hearing Screening, please fill out the Mandatory Vision and Hearing Screening Opt Out form and bring it to the Health Office by October 22, 2018. El Departamento de Educación de California requiere la evaluación obligatoria de la vista y la audición de todos los estudiantes en los grados Kinder, 2º, 5º, 8º y 10º. Debido a que la vista y la audición impactan en gran medida la capacidad de aprendizaje de un estudiante, el Estado de California requiere estas evaluaciones para garantizar que cada alumno pueda acceder completamente a su educación.  

Las Enfermeras del Distrito de San Marcos llevarán a cabo la evaluacion obligatoria de la vista  y Audición en la escuela de su hijo la próxima semana. Si su hijo no pasa la prueba de vista o audición, se le notificará por carta. Si su hijo pasa, usted no será notificado. Si no desea que su hijo participe en la evaluación obligatoria de la vista y la audición, complete el formulario de exclusión del examen obligatorio de la vista y la audición disponible en el sitio web de la escuela y llévelo a la Oficina de Salud antes del 22 de octubre de 2018.
