Athletic Clearance Information
- Students who wish to participate in soccer for the 26/27 school year must have an Online Clearance Packet completed and turned in before you can try out.
- Sports Physicals may only be performed by a licensed MD, DO, PAC, and NP.
- The clearance packet is electronic and all physicals must be uploaded online. (No physical paperwork is collected.)
- Clearance Packet Due 11/1: Clearance & Insurance
